FORMS All forms listed below must be completed and
handed in order to register your child for football or cheer activities. Please
note the following: ü Your child will need to have a current
physical and the medical form must be signed and dated by your child’s
physician. ü You will need to provide a copy of your
child’s birth certificate. ü You will need to provide a copy of your
child’s school report card from the 2006/2007 school year. |
